Plasmodium vivax is the most widespread parasite causing malaria, being
especially prevalent in the Americas and Southeast Asia. Children are one of the
most affected populations, especially in highly endemic areas. However, there are
few studies evaluating the therapeutic response of infants with vivax malaria.
This study retrospectively evaluated the parasitaemia clearance in children
diagnosed with vivax malaria during the first five days of exclusive treatment
with chloroquine (CQ). Infants aged less than six months old had a significantly
slower parasitaemia clearance time compared to the group of infants and children
between six months and 12 years old (Kaplan-Meier survival analysis; Wilcoxon
test; p = 0.004). The impaired clearance of parasitaemia in younger children with
vivax malaria is shown for the first time in Latin America. It is speculated that
CQ pharmacokinetics in young children with vivax malaria is distinct, but this
specific population may also allow the detection of CQ-resistant parasites during
follow-up, due to the lack of previous immunity.相似文献
The National Longitudinal Study of Adolescent Health data were used to test predictions from life history theory. We hypothesized
that (1) in young adulthood an emerging life history strategy would exist as a common factor underlying many life history
traits (e.g., health, relationship stability, economic success), (2) both environmental harshness and unpredictability would
account for unique variance in expression of adolescent and young adult life history strategies, and (3) adolescent life history
traits would predict young adult life history strategy. These predictions were supported. The current findings suggest that
the environmental parameters of harshness and unpredictability have concurrent effects on life history development in adolescence,
as well as longitudinal effects into young adulthood. In addition, life history traits appear to be stable across developmental
time from adolescence into young adulthood.

We surveyed the diversity of cultivable fungi isolated from cold and hot volcanic soils of Deception Island, Antarctica. Seventy-four fungal isolates were identified; these belonged to 17 taxa in the genera Aspergillus, Penicillium, Pseudogymnoascus, Purpureocillium, and Mortierella. The fungal assemblages showed low diversity, richness, and dominance indices. The Aspergillus taxa were dominant in the soils at 0 °C, 50 °C, and 100 °C. Aspergillus lacticoffeatus, Aspergillus cf. ruber, Penicillium citrinun, and Purpureocillium sodanum were present only in soils having a temperature of 100 °C. Aspergillus calidoustus was present in all thermal soils and displayed the highest densities. The majority of fungi displayed mesophilic behavior; however, different isolates of Aspergillus lacticoffeatus and Aspergillus niger were able to grow at 50 °C; these are phylogenetically close to the causative agents of aspergillosis in immunocompromised individuals. Deception Island perhaps represents one of the most visited regions in Antarctica and the tourism there has increased over the last 20 years, especially by elderly tourists, probably with weak immune systems, come in contact with the resident microorganisms, including the thermo-resistant opportunistic Aspergillus species.
Neonatal infections cause a significant proportion of deaths in the first week of life, yet little is known about risk factors and pathways of transmission for early-onset neonatal sepsis globally. We aimed to estimate the risk of neonatal infection (excluding sexually transmitted diseases [STDs] or congenital infections) in the first seven days of life among newborns of mothers with bacterial infection or colonization during the intrapartum period.
Methods and Findings
We searched PubMed, Embase, Scopus, Web of Science, Cochrane Library, and the World Health Organization Regional Databases for studies of maternal infection, vertical transmission, and neonatal infection published from January 1, 1960 to March 30, 2013. Studies were included that reported effect measures on the risk of neonatal infection among newborns exposed to maternal infection. Random effects meta-analyses were used to pool data and calculate the odds ratio estimates of risk of infection. Eighty-three studies met the inclusion criteria. Seven studies (8.4%) were from high neonatal mortality settings. Considerable heterogeneity existed between studies given the various definitions of laboratory-confirmed and clinical signs of infection, as well as for colonization and risk factors. The odds ratio for neonatal lab-confirmed infection among newborns of mothers with lab-confirmed infection was 6.6 (95% CI 3.9–11.2). Newborns of mothers with colonization had a 9.4 (95% CI 3.1–28.5) times higher odds of lab-confirmed infection than newborns of non-colonized mothers. Newborns of mothers with risk factors for infection (defined as prelabour rupture of membranes [PROM], preterm <37 weeks PROM, and prolonged ROM) had a 2.3 (95% CI 1.0–5.4) times higher odds of infection than newborns of mothers without risk factors.
Conclusions
Neonatal infection in the first week of life is associated with maternal infection and colonization. High-quality studies, particularly from settings with high neonatal mortality, are needed to determine whether targeting treatment of maternal infections or colonization, and/or prophylactic antibiotic treatment of newborns of high risk mothers, may prevent a significant proportion of early-onset neonatal sepsis.

To determine the period of harvest that optimizes the antimicrobial activities of the essential oil of Ocimum gratissimum L. from Benin, aerial plant parts were collected at two vegetative stages (pre- and full-flowering) and three sampling times (7?am, 1?pm, and 7?pm). Extraction by hydrodistillation yielded between 0.65 and 0.78% of essential oils. Characterization of the oils by GC-FID and GC/MS analysis revealed the presence of monoterpenes (87.26-93.81%), sesquiterpenes (5.57-11.34%), and aliphatic compounds (0.15-0.18%), with p-cymene (1; 28.08-53.82%), thymol (2; 3.32-29.13%), γ-terpinene (3; 1.11-10.91%), α-thujene (4; 3.37-10.77%), and β-myrcene (5; 4.24-8.28%) as major components. Two chemotypes were observed, i.e., a p-cymene/thymol and a p-cymene chemotype, for plants harvested at 7?am for the former and at 1?pm or 7?pm for the latter, respectively. The oils were fungicidal against Candida albicans, with the sample from full-flowering plants collected at 7?am being the most active (MIC?=?0.06±0.00?mg/ml). The chemical variation of the oils also influenced the antimicrobial effect against Staphylococcus aureus; the most active oil was obtained from plants at the pre-flowering stage collected at 7?am (MIC=0.24±0.01?mg/ml). Escherichia coli was insensitive to the chemical variation of the oils (MICs of ca. 0.48±0.02?mg/ml for all oils). Moreover, the essential oils showed low toxicity against Artemia salina Leach larvae, with LC(50) values in the range of 43-146?μg/ml. Choristomas are lesions composed of normal cells or tissues occurring in an abnormal location. Cartilaginous choristomas of the oral mucosa are rare and occur preferentially on the tongue and less often in sites such as the soft palate and gingiva. Oral lesions are generally covered by integral mucosa and can occur at any age. The present study describes a case of a 73-year-old female presenting with an asymptomatic cartilaginous choristoma on the ventral surface of the tongue which had developed over a period of 3 years. The clinical presentation and management of the case are discussed and the literature is reviewed. This is the 28th reported case of a cartilaginous choristoma of the tongue and the third with a ventral localisation. 相似文献
IL4, the gene coding the prototypic Th2 cytokine, has been frequently studied in the context of several inflammatory conditions, but conclusive results have not been obtained. This gene is located in the 5q31-33 complex genetic region, which shows some susceptibility factors to type 1 diabetes (T1D) and rheumatoid arthritis (RA) among other inflammatory conditions. Our aim was to assess the involvement on T1D and RA of IL4 polymorphisms considered individually and in combination with other polymorphisms in 5q31-33, specifically in the OCTN locus, where the L503F polymorphism has been associated with Crohn's disease and other Th1 diseases. We performed a case-control study including 316 T1D patients, 599 RA patients and 540 healthy controls, all of them corresponding to white Spanish individuals. The IL4 single-nucleotide polymorphisms (SNPs) -590C/T (rs2243250) and the OCTN1 exonic SNP L503F (rs1050152) were analysed in all samples. Frequency comparisons of -590C/T and stratified analysis including both cited SNPs were performed using chi-square tests. The -590C/T IL4 SNP was not found associated with T1D or RA when individual analyses were performed. However, a significant association with T1D emerged after stratification by L503F [p=0.02, odds ratio=1.95, 95% CI=1.07-3.55]. The location of the IL4 gene in the complex 5q31-33 genetic region, which contains many genes involved in immunological responses and presents linkage disequilibrium extended along many kilobases, makes necessary to interpret cautiously the previous IL4-association studies. 相似文献
